Distal Jet

The Distal Jet is constructed around a highly efficient piston, and tube principle, coupled with a controlled and defined force delivery. The Distal Jet is uniquely adapted to a bio-mechanic approach to maximize outcomes. Superior distalizer combines simple translation with excellent anchorage control and positive, simple molar retention with one appliance. Typical clinical situations for the Distal Jet are: Class II maxillary protrusion, Class II molar relations resulting from missing or blocked out teeth, and Class II subdivision (unilateral) cases. Expansion can also be achieved by adding a transverse screw.

Treatment & Care

  1. Remove separators and clear any debris from the interproximal areas. Seat the appliance completely, checking the fit before cementation. Check the passive fit of directors and bayonets – adjust as necessary.
  2. Mix cement, load bands and cement the appliance as a single unit, in a customary manner.
  3. After cement clean-up, remove stabilizing floss or wire before activation.
  4. The appliance is activated initially after cementation and at four-week intervals by complete compression of coil spring with the activation lock (see special note below).

How to Convert to a Nance

  1. After distalization is complete, convert the Distal Jet to a retainer.
  2. Peel the spring from the appliance with a Weingart plier.
  3. Slide the lock firmly against the stop and tighten the screw.
  4. Squeeze the end of the lock tightly onto the bayonet wire. Important: this prevents the appliance from becoming loose.
  5. An alternative option is to tie the lock and bayonet wire/molar sheath together with a steel ligature or elastic chain or thread.

Special Note

Molar rotations must be corrected before they are distalized. This is easily accomplished in the lab by bending rotational compensation into the double-back sections of the bayonets. Seat and cement as above, do not activate and recall in one month. Begin activation after molars are corrected.

This appliance should be cleaned every day with toothpaste and a soft toothbrush, morning and night. Encourage the patient to give sufficient time to clean thoroughly around the appliance, gums, teeth, and bands to maintain proper hygiene and prevent inflammation. 3 minutes is recommended.

Dr. Aldo Carano and Dr. Mauro Testa invented the Distal Jet appliance in 1996. The Distal Jet is a fixed appliance used in orthodontics to move molars backward, or distally, to create space in the dental arch.

The Distal Jet was designed to address some of the disadvantages of other distalization appliances, including:

  • Molar tipping
  • Most distalization methods apply force at the crown level, which can cause the molars to tip. The Distal Jet applies force closer to the center of resistance of the tooth, which reduces tipping.
  • Patient cooperation
  • The Distal Jet can produce molar distalization without relying on patient cooperation.

The Distal Jet is a combination of NiTi springs and set screws. After molar distalization, the appliance can be converted into a nance style appliance to hold the molars in place.
