MARPE stands for Miniscrew-Assisted Rapid Palatal Expander. It is an orthodontic appliance used to support maxillary expansion with the help of temporary anchorage devices, or TADs.
Unlike traditional tooth-borne expanders, MARPE appliances use skeletal anchorage to help improve control during expansion. The exact design, TAD position, workflow, and activation protocol should always be determined by the treating doctor based on the patient’s anatomy and treatment plan.

MARPE is the broader category of miniscrew-assisted expansion appliances. MSE is one specific style of MARPE appliance. ODL offers multiple MARPE options, including MSE, Custom Leone MARPE, Custom Power Screw MARPE, and other TAD-borne designs.

TAD-first means the TADs are placed first, then the patient is scanned, and the appliance is designed to fit the actual TAD positions. This is often useful when accuracy is critical or when the patient’s anatomy requires a more controlled approach.
TAD-last means the appliance is fabricated first, then the TADs are placed through the planned eyelet positions at delivery. This can be efficient for more straightforward cases.
Hybrid combines guided TAD placement with immediate appliance delivery. This can reduce the number of appointments while still supporting a more planned placement workflow.

CBCT + STL overlay helps the doctor and design team evaluate the relationship between the teeth, palate, bone, and appliance design.

Overlaying the CBCT with the scan helps support better planning, better communication, and a more informed approval process before manufacturing.
Yes. If you are unsure which appliance or workflow is best for a case, ODL can assist through live planning or a digital approval workflow.
Doctors can submit the records, indicate that they would like ODL to recommend the setup, and review the proposed design before the appliance is manufactured. This allows the treating doctor to maintain final approval while getting support from the ODL team during planning.
Yes. Every MARPE case requires doctor approval before fabrication.
Once the digital workup is ready, the doctor will receive a notification to review the design. The case does not move into manufacturing until the prescribing doctor approves the setup. This step helps confirm that TAD positioning, appliance style, screw selection, and overall design match the treatment plan.
Timeline depends on the workflow, case complexity, doctor approval speed, and whether all required records were submitted correctly.
As a general expectation, MARPE cases should be planned with enough time for:

For a first MARPE case, we recommend submitting complete records and clearly indicating your preferences—or letting us know that you would like ODL to recommend the setup.
A strong first submission should include your scan, CBCT, preferred TAD system if known, preferred workflow, and any clinical notes. From there, ODL can support the case through live planning or digital notification approvals so the doctor feels comfortable before manufacturing begins.
